Hepatic synthesis of BA from cholesterol involves entry into hepatocytes of lowdensity lipoprotein (LDL) cholesterol by binding to LDL receptors on the hepatocyte cell surface. Upregulation of hepatic BA synthesis promotes maintenance of BA pool size and leads to an increase in serum 7 α-hydroxy-4-cholesten-3-one (C4), a surrogate for the activity of cholesterol 7 α-hydroxylase, the rate-limiting enzyme in hepatic BA synthesis.
