2	Increase in serum creatinine of >200–300% (>2 to 3-fold) above baseline	<0.5 mL/kg/hr for >12 hours
3	Increase in serum creatinine of >300% (>3-fold) above baseline or a serum creatinine ≥4 mg/dL with an acute rise of ≥0.5 mg/dL	<0.3 mL/kg/hr for 24 hours or anuria for 12 hours
