As described, according to clinically measured height at weight. If a
height measurement was not available it was estimated from the mean of
two measurements of knee to heel measurements, if these were
obtained, with the following formula as has been recommended [30]:
Estimated height = (1.83 × knee height, cm) − (0.24 × age) + 84.88
