Clinician and Patient Eligibility Criteria

Patient Participants	Currently enrolled patients Scheduled for a routine, follow-up, or health maintenance office visit Scheduled to see a participating clinician 18 years of age or older Able to provide written informed consent Have no medical contraindications to moderate physical activity	Have a life-threatening or acutely unstable medical problem which precludes participation Unable to read and understand English
Clinician Participants	Practicing clinicians (physicians, physician assistants, or nurse practitioners) at study site	Planning to move or relocate to another practice in the study period
