Prevention of Intraventricular hemorrhage

A. Prenatal interventions:
  1. Prevent preterm delivery

  2. Maternal transport to regional neonatal center

  3. Prenatal glucocorticoids

  4. Agents of unproven benefit:

    Prenatal phenobarbitone, vitamin K, and magnesium sulfate

B. Care during infant delivery: Optimize obstetric care and prevent prolonged labor
C. Postnatal interventions:
  1. Optimize neonatal resuscitation

  2. Reduce fluctuation in cerebral blood flow:

    Minimize handling and suctioning, synchronized and gentle ventilation, prompt treatment of patent ductus arteriosus, maintaining normal O2 and CO2, preventing apneic episodes and seizures.

  3. Correction of coagulation and bleeding disorders

  4. Indomethacin:

    Reduces IVH, but does not enhance long term outcome.

  5. Agents of unproven benefit:

    Phenobarbitone, vitamin E, etamsylate
