Hypoxia/hypoxemia + intrapulmonary shunting	Hypoxemia defined as: PaO₂ <70 mm Hg on room air or Pulse oximetry ≤96%¹³ (room-air or supplemental O₂)
	Intrapulmonary shunting (right→left bubbles on echocardiogram after 3 cardiac cycles and/or free text stating “intrapulmonary shunting”)
