Data indicate no. (%) of patients. Total no. of cases was 1,024, because detailed clinical information was not available for 174 patients. Charlson’s weighted index of co-morbidity and each underlying diseases were analyzed in separate logistic regression models to avoid data overlap in the multivariate analysis.
