	Locate and order evidence-based interventions for chronic disease management, surveillance, and comorbidity prevention for major chronic disease diagnoses	Chronic diseases (e.g., diabetes, chronic obstructive pulmonary disease, hypertension, hyperlipidemia) and other active medical problems have an appropriate evidence-based plan of care succinctly documented in the EHR comments section.
	Use a health maintenance tool for tracking routine prevention and chronic disease surveillance activities	Based on the patient’s age, gender, and personal risks, and using the U.S. Preventive Services Task Force recommendations, the health maintenance tab in the EHR accurately reflects all indicated routine screening activities for the prevention and surveillance of disease, timed at the correct intervals.
Demonstrates the ability to order prescriptions, laboratory tests, vaccines, imaging studies, and consultations (MK, PC)	Place orders for indicated medications based on the patient’s medical issues	Orders for new medications are placed in the EHR based on evidence based-guidelines for each diagnosis.
