A 52-year-old female fell down a flight of four steps. CT was obtained after cardiac arrest to evaluate for a pulmonary embolism. There is a smooth conical aortic outpouching (arrows) surrounded by a soft tissue mass. This patient has a ductal remnant adjacent to a left hilar small cell lung carcinoma, and it is important not to mistake this for a traumatic aortic injury with adjacent mediastinal hematoma.
