Age is assessed per 10-year increase; systolic blood pressure per 20 mm Hg increase; total cholesterol per 20 mg/dL (0.52 mmol/L) increase; high density lipoprotein (HDL) cholesterol per 10 mg/dL (0.26 mmol/L) increase; and glomerular filtration rate (GFR) per 5 mL/min/1.732 (0.08 mL/sec/1.73m2) increase.
