Moderate risk
• PICU patient

• Doubling of SCr, OR

• eCrCl decrease >50 %, OR

• Fluid overload >15 %

High risk
• Acute, decompensated heart failure

• Serum Cr increase >0.3 mg/dl, OR

• eCrCl decrease 25–50 %,

• Fluid overload >10 %

• Stem-cell transplant
Very high risk
• Receiving mechanical ventilation and one or more vasoactive medications

• Any serum Cr increase

• eCrCl decrease >25 %,

• Fluid overload >5 %
