Similar to Figure 1, this chart shows the process that the respondent perceived to be responsible for ongoing provision of inappropriate life-prolonging treatment. The respondent was asked if ongoing provision of this treatment is the result of (pick any): 1. Failure of communication before intensive care unit (ICU) admission; 2. Failure of communication on or after ICU admission; 3. Concern regarding medicolegal implications; 4. Process to address care STARTED – ongoing testing and/or decision making is pending; 5. Other, for example, ‘continuing for a few days’
