Additional Guidelines in UCSF Down-Staging Protocol

  1. A minimum follow-up period of 3 months after down-staging is required before deceased donor liver transplantation (DDLT) or LDLT, along with imaging studies meeting the above defined criteria for successful down-staging.

  2. Approval by the regional UNOS review board for priority listing is needed for DDLT after successful down-staging.

  3. Patients can undergo LDLT if a donor is available, and if imaging studies meet proposed UCSF expanded criteria.10

  4. Those with acute hepatic decompensation after down-staging procedures are not eligible for DDLT or LDLT unless they meet the above criteria.

  5. In patients treated with resection as a down-staging procedure, the presence of microvascular tumor invasion in the resection specimen is a contraindication to DDLT or LDLT.
