- Acute myeloid leukemia

- Chronic myeloid leukemia

- Myelodysplastic syndrome

(2) Certain conditioning therapies, particularly:

- Busulphan

- Cyclophosphamide

- Total body irradiation

(3) Corticosteroid therapy

(4) T-cell depletion in allografts

(5) Matched unrelated allogeneic HSCT

(6) Mismatched allografts

(7) Acute and chronic graft versus host disease

(8) Bronchiolitis obliterans

(9) History of M. tuberculosis infection
