Interventions | Early discharge community based Type of service: average of 2.5 home visits a week for 3 months, each visit lasting 45 minutes. Multidisciplinary meetings held to discuss the assessment of patients and progress towards rehabilitation goals, which were set by relatives, patient and therapist.
