CT images at other levels of the larynx were normal. Clinical history mentioned recently diagnosed left vocal cord paralysis confirmed by fibre optic endoscopy. This compensatory increased uptake in the right vocal cord was due to recent left vocal cord paralysis. This case illustrates the need for rigorous correlation with clinical findings whenever the only abnormality observed is unilateral vocal cord FDG uptake
