During the study period (April 2010–December 2012), there were 1,822,519 adult patients. We excluded 690,933 patients (249,442 did not have at least 3 outpatient visits during this time; 203,759 did not have at least 3 EVS assessments; 165,933 had a Charlson’s index above 0; 71,127 were pregnant during the study period; and 672 had a temperature above 99.9 degrees). This left us with a study sample of 1,131,586 members. After excluding members who were taking antihypertensive medications (461,585), those taking glucose-lowering medications (42,783), and those who were underweight (4,321), data from 622,897 patients were available for analyses.
