* Hazard rate ratios adjusted for sex (as a strata variable) and for education (≤ high school, some college, college and above), body mass index (<25kg/m2, 25-<30kg/m2, ≥30kg/m2), smoking status (never, former, current), and alcohol intake (nondrinkers, <2 drinks/day, ≥2 drinks/day) by Cox regression using age as the time metric.
