Figure.

The first randomization in the FIT Families intervention took place at the beginning of the first 3 months of the intervention. Participants were randomized into 1 of 2 groups: 1) Home Motivational Interviewing and Cognitive Behavioral Therapy or 2) Office Motivational Interviewing and Cognitive Behavioral Therapy. If the participant in the home program lost 3% or more of his/her weight, then the participant was placed in the home maintenance program during the second 3 months of the program. A second randomization took place among participants who lost less than 3% of their weight. These participants were randomized to either a home skills program or home contingency management. Similarly, in the office-based program, a second randomization took place. If the participant in the office-based program lost 3% or more of his/her weight, then the participant was placed in the office maintenance program during the second 3 months of the program. A second randomization took place among participants who lost less than 3% of their weight. These participants were randomized to either a home skills program or home contingency management.

Overview of sequential multiple assignment randomized trial (SMART) design (19) for the FIT Families study, Michigan, 2010–2014. The first randomization took place at baseline and the second at 3 months. The second randomization took place if the adolescent participant did not lose 3% of his or her initial body weight. “Office” refers to the Wayne State University Pediatric Weight Management Center.
