Stage SCr Criteria Urine Output Criteria
Stage 1 Increase in SCr >0.3 mg/dL or SCr 1.5–2x baseline <0.5 mL/kg/hr for 6 hours
Stage 2 SCr 2–3x baseline <0.5 mL/kg/hr for 12 hours
Stage 3 SCr >3x baseline, or SCr >4 mg/dL with an acute rise
of >0.5 mg/dL, or receiving renal replacement therapy		 <0.3 mL/kg/hr for 24 hours or anuria for 12 hours
