aThe restrictive pattern is forced volume capacity (FVC) and forced expiratory volume in 1 second (FEV1) values of less than 80% predicted and an FEV1/FVC ratio greater than 80%.

bThe obstructive pattern is a FEV1/FVC ratio of less than 67%.

cThe Haller index is calculated as the inner transverse thoracic diameter divided by the anteroposterior distance between the anterior thoracic wall and the spine at the narrowest point.
