The model included age, sex, race/ethnicity, annual household income, education level, region of residence, body mass index, smoking status, diagnosis of diabetes, and diagnosis of high blood pressure.
Responses were offered on a 5-point Likert scale of “strongly agree,” “moderately agree,” “somewhat agree,” “neither agree nor disagree” and “disagree.” “Strongly agree,” “moderately agree,” and “somewhat agree” were combined into 1 “agree” category.
Questions were answered as yes or no.
