A 79-year-old woman with known basilar tip aneurysm presenting with acute onset of severe headache (same patient as in Figure 1). The non-contrast CT scan study demonstrates diffuse subarachnoid hemorrhage in the perimesencephalic cistern and left ambient cistern extending along the left tentorium, consistent with rupture of basilar tip aneurysm.
