Severe tilting/canting of the transcatheter valve (yellow arrows) occurred in the setting of operator-independent motion of the valve during balloon inflation (A). This malpositioning prevents normal closure of the valve, resulting in severe central aortic regurgitation (B, red arrows). After placement of a second transcatheter valve (valve-in-valve salvage), there was no central regurgitation but moderate paravalvular regurgitation was seen (C, blue arrows).
