Case 2, intraoperative findings. (a) The left foramen of Monro was firmly obstructed by inflammatory substances that represented an obstacle for foraminoplasty (arrow). (b) A thin, semi-transparent portion (arrow) of septum was identified posterior to the anterior septal vein. Fenestration was performed in this portion using the techniques mentioned in Figure 2.
