Traumatic insertion with displacement of basilar membrane. As indicated in A, the image reconstruction suggests that the array straddles the scala tympani (ST) and scala vestibuli (SV). Microdissection revealed a traumatic insertion with electrode buckling in the area of the cochleostomy (B), as well as displacement of the basilar membrane above the level of its normal attachment with the lateral bony wall (arrows). The array thus sits below the basilar membrane but in approximately the region that the membrane would normally occupy, with the apical tip situated more superiorly within the SV.
