Model 1 adjusts for age, sex, race, and clinic site.

Model 2 additionally adjusts for smoking, body-mass index, alcohol consumption, prevalent coronary heart disease, physical activity, and estimated glomerular filtration rate.

Model 3 further adjusts for history of falling.
