Example of the hierarchical SCT in cubital tunnel syndrome. This figure illustrates the sequence of collapse of the five anatomic sites of ulnar nerve compression tested in this study, in a patient with mild cubital tunnel syndrome (corresponds to Video, Supplementary Digital Content 1, https://vimeo.com/59943112). In this patient, the primary site of collapse was the cubital tunnel retinaculum just posterior to the medial epicondyle, and the remaining sequence was identical to that found in all study patients. Additionally, this patient demonstrated collapse at the thoracic outlet and parascapular muscles once the initial five compression sites in the arm, forearm, and hand had been “frozen out” with ethyl chloride
