K2 and w3, reported in the 3rd specification, are the averages of individualized K2 and w3. For estimating k2 and w3 for each individual, we regressed (ΔCortisolΔt) against 17 variables for ACTH and 17 variables for cortisol. The estimated k2s and w3s are listed in Table 3 of the online appendix. The program that we wrote to estimate these parameters are included in the online appendix.
