  • Total hysterectomy for benign causes

  • Pregnant or having first postpartum visit during time period

  • Papanicolaou test preformed outside of the family practice unit

  • STI testing declined

  • Required STI screening before IUD insertion

  • Incident prompting STI screening (eg, condom breaking)

  • Congenital absence of uterus

  • Clear documentation of having never been sexually active
