| 3 | Well, with treated comorbid disease |
| 4 | Apparently vulnerable (not frankly dependent, disease symptoms, slowed up) |
| 5 | Mildly frail (limited dependence in IADL) |
| 6 | Moderately frail (help needed with ADL and IADL) |
| 7 | Severely frail (completely dependent in ADL, or terminally ill) |
