End Stage Liver Disease and one or more of the following:

• End Stage Renal Disease requiring hemodialysis

Or

• Chronic Kidney Disease with GFR ≤30 mL/min* for >3 months

Or

• Kidney biopsy showing >30% glomerulosclerosis or interstitial fibrosis

Or

• Acute Kidney Injury with creatinine ≥2.0 mg/dL and/or requiring dialysis for ≥8 weeks
