Men: <36.0, 36.0–41.5, 41.6–46.5, 46.6–52.0, >52.0 (measured quintiles); and <37.1, 37.1–41.6, 41.7–46.0, 46.1–51.3, >51.3 (predicted quintiles). Women: <29.1, 29.1–33.3, 33.4–37.3, 37.4–42.1, <42.1 (measured quintiles), and <30.5, 30.5–33.8, 33.9–37.1, 37.2–41.2, >41.2 (predicted quintiles).
