MDQ = Mood Disorder Questionnaire. Those studies recruiting from secondary care are not included unless data are given separately for primary care. Patients with recognised bipolar disorder are removed from the denominator. Where data are supplied, the rate for bipolar I and II only is used, rather than the rate for the broader bipolar spectrum.
