In the first task-free fMRI study of inter-regional connectivity in essential tremor, Gallea et al. reveal that the supplementary motor area has decreased functional connectivity with the M1 hand area, and also shows increased grey matter and corticospinal projections. Changes correlate with clinical severity and cerebellar grey matter loss.
