Three of the 5 outbreaks in Puerto Rico occurred in the same cave system and appear as a single point on the map. Histoplasmosis is a reportable disease in Arkansas, Delaware, Illinois, Indiana, Kentucky, Michigan, Minnesota, Nebraska, Pennsylvania, and Wisconsin.
