Age 21 years or older
Chronic stroke phase (>6 months to 20 years post stroke)
Unilateral stroke in brain/midbrain
Ability to extend fingers, thumb, wrist at 10° or above
Complaint of ability to use paretic hand

Diagnosis of TPS (>6 months)
MRI-confirmed lesion in the thalamus or above/below the thalamus
Lesion in the semioval white matter in topography consistent with TPS
Complaint of intractable hemibody pain, severe allodynia, or pain-related disability
Failure to respond to at least 1 antidepressant, antiseizure medication, or narcotic

Cardiac implants
Metallic implant in the head
Seizure disorder
Medication-resistant epilepsy in first-degree relative
Neuro-/psychoactive medication
History of fainting spells
Pregnancy
Implanted pumps, stimulators, and shunts
Sensorimotor system condition
Mini-mental exam score lower than 18
Ongoing rehabilitation for the upper limb
MRI incompatibility
Not interested or unable to reach

Neurological lesions beyond TPS lesion
Pregnancy or inadequate contraception
Certain serious comorbidities
Unable to provide informed consent
