All models control for patient age, gender, race, education, Charlson comorbidity score, Framingham cardiovascular disease risk, depressive symptoms, body mass index (BMI), smoking and drinking status, number of preventive services due; physician age, gender, race, and specialty; and the gender and race concordance of the patient–physician dyad, whether the patient had seen the same physician in the prior year, and the minutes after the scheduled appointment time the visit started.
