Chronic Care Quality Measures:

  1. Increased Activities of Daily Living (ADL) dependency

  2. Moderate-severe pain

  3. Pressure sores

  4. Physical restraints

  5. Bowel-bladder incontinence and use of indwelling catheters

  6. Most of time in bed or chair

  7. Decreased ability to move

  8. Urinary tract infection

  9. Increased depression-anxiety

  10. Weight loss

Postacute Care Quality Measures:
Percentage of short stay residents with
  1. Delirium

  2. Moderate-severe pain

  3. Pressure sores
