Identify patients without caregivers as high risk for having difficulty accessing and taking medications.
Reasons for Hospital Admission	Increase patient education efforts on awareness of symptoms. Specify an action plan for patients to use as soon as symptoms start. Educate clinicians on patient and caregivers perspectives on reasons for hospital admission.
