  • Incorporate the assessment of patient-identified HF management challenges, such as limited functional status, activities of daily living, and stress, in a structured way into clinical encounters.

  • Identify patients without caregivers as high risk for having difficulty accessing and taking medications.

Reasons for Hospital Admission
  • Increase patient education efforts on awareness of symptoms.

  • Specify an action plan for patients to use as soon as symptoms start.

  • Educate clinicians on patient and caregivers perspectives on reasons for hospital admission.
