Canagliflozin is limited to 100 mg once daily in patients who have an eGFR of 45 to <60 mL/min/1.73 m2
Do not initiate canagliflozin if eGFR is <45 mL/min/1.73 m2
Discontinue canagliflozin if eGFR falls persistently below 45 mL/min/1.73 m2
