Standard dosage 15 mg/kg in adults if administered daily and 25–30 mg/kg if administered twice or thrice weekly with a maximum dose of 1.5 g.
Contraindications Hypersensitivity to the drug.
Major side effects Vestibular ototoxicity, facial paresthesias, rash, fever, urticaria, angioneurotic edema, and eosinophilia.
Major drug interactions Ototoxic effects accentuated with co-administration of ethacrynic acid, furosemide, mannitol and possibly other diuretics.
Special points Category D drug in pregnancy.
Cost Inexpensive.
