Indications of rFVIIa and plasma and PRBC (1:1 ratio)

Clinical conditions
Bleeding not controlled Truncal/axillary/neck or groin
with tourniquets or local haemostatic dressings (‘quickclot’) bleeding
Proximal amputation or mangled extremity Large soft tissue injuries
Chest tube drainage > 1000 cc blood total
> 200 cc/hr for 4 consecutive horns
Physical exam findings Decreased mental status from injury and shock Severe head injury Clinically coagulopathic
Objective physical exam or INR ≥ 1.5
Laboratory findings BD ≥ 6
Haemoglobin - < 11g/dL in young Hypothermic from blood loss (T<96°F)
Hypotensive from blood loss (SBP < 90 mmHg) or a weak/absent radial pulse
Need for fresh whole blood Bilateral proximal amputations
Transfusion Large hemoperitoneum and significant shock
