Survival curves plotted from the multivariate analysis that included age, sex, New York Heart Association functional class, left ventricular ejection fraction, etiology of heart failure, diabetes, and treatment with β‐blockers, angiotensin‐converting enzyme inhibitors‐angiotensin II receptor blockers, and mineralocorticoid receptor antagonists as covariates. Thirteen patients were excluded from the analysis because of unknown cause of death.
