Transgluteal approach is preferred for the majority of presacral masses (arrow). The medial and inferior approach avoids injury to the sciatic nerve and superior and inferior gluteal vessels and avoids traversal of the piriformis muscle, which can be painful. This is ideally performed in the prone position.
