Transsacral (light blue arrow) approach is usually reserved for osseous lesions, which may extend to the presacral space. The approach is medial to the neural foramen and lateral to the spinal canal. The extraperitoneal anterolateral approach (yellow arrow) can target a pelvic sidewall lymph node or mass (red oval) with a course through the iliacus muscle. For colour image see online.
