^aL-asparaginase was given at the discretion of the clinician at a dose of 400 IU kg⁻¹ SQ or 2500 IU cat⁻¹ SQ.

^bVincristine or vinblastine was given at the discretion of the prescribing clinician. Vincristine was given at 0.5–0.7mgm⁻², IV or vinblastine was given at 1.5mgm⁻², IV.

^cCyclophosphamide (Cytoxan) was given 200–250mgm⁻² IV or PO.

^dDoxorubicin was given at 1mg kg⁻¹ or 25 mgm⁻² IV.

^ePrednisone or prednisolone was given at 1–2mg kg⁻¹ PO q24 h continued through the protocol to be tapered upon completion. Tapering occurred earlier at the discretion of the clinician.
