Table 1.

Mitral Stenosis Qualitative Wilkins Score

Score Leaflet mobility Valve thickening Calcification Subvalvular thickening
1 Highly mobile Near normal (4–5 mm) Single area of brightness Minimal chordal thickening
2 Reduced mobility Thickened tips Scattered areas at leaflet margins Chordal thickening up to one-third
3 Basal leaflet motion only Entire leaflet thickened (5–8 mm) Brightness extends to midleaflets Distal third of chordae thickening
4 Minimal motion Marked leaflet thickening (>8–10 mm) Extensive leaflet brightness Extensive thickening to pap muscle

The mitral valve morphology is considered favorable if the mitral echocardiographic score is ≤8.

Wilkins score = Leaflet Mobility + Valve Thickening + Calcification + Subvalvular Thickening.
