Electroanatomical endocardial 3D mapping shows a large diseased area in the antero-septal portion of the RVOT (<0.5 mV), where fragmented low-voltage electrograms are detected (Panel A, left and right), diagnostic of arrhythmogenic right ventricular cardiomyopathy/dysplasia. VT 12-lead ECG (Panel B) and intracardiac electrograms (Panel C) are shown: radiofrequency delivery at the site of interest, where a diastolic fragmented activity is recorded, immediately terminates the VT.
