CPD, pack years, and years since quit smoking was analyzed as a continuous variable representing the number of cigarettes smoked per day, number of multiplying years of smoking by the number of cigarettes divided by 20, years since quit smoking in former smokers, respectively. Smoking initiation and smoking cessation were analyzed as dichotomous variables, contrasting ever versus never and former versus current smokers, respectively.

Analyses were adjusted by age, sex and fve EVs.
